Travel Permit Application - Tournaments

Travel permits are required for all tournaments - both home and away. Tournaments outside of the OMHA are subject to a $20 fee. Fees are to be paid in CASH or credit card in the TNT Office and must accompany this form. Applications will not be processed without payment. Receipts will be given.
